The company was formed in 2008 by seven healthcare professionals involved in the areas of acute care hospital, rehab hospital, skilled nursing and rehab, out-patient clinics, and Home Health Care. The company's management team consists of healthcare professionals with excellent clinical skills, supervisory skills and business experience. The company's president and administrator has been a clinician since 1992 covering acute care to home health, was a Director or Rehabilitation for many years in a sub-acute skilled nursing facility (SNF) for one of the nation's largest chain of SNF and LTC, and has been the Vice-President of Operations for the past 5 years for a well known healthcare staffing agency in the city.
Having fully experienced the whole spectrum of rehabilitation from acute-care to home health services, we have noticed that some break down in the continuity of rehabilitation can occur as patient transitions from one setting to another. This may happen partly because of workers who are not able to comprehend the overall picture of each patient's clinical pathway to recovery, or the lack of staff with adequate experience, or the lack of dedicated and caring staff, or the shortage on home health workers who have joined the profession with the right mission and purpose.
